the ParQ Form
and ParMedX Form
Although the ParQ is indicated as covering up to age 69, it is fine for all age groups and we can use it for those older than 69. It identifies when one should consult with a doctor before starting an exercise program.
Doctor consultation from participants is NOT required to participate in the class; but for those who answer ‘yes’ to questions on the ParQ, the form helps them to be aware of when they should check with their doctor and be cautious about exercising.
For those with medical conditions, they may want to take the ParMedX form to their doctor if they wish. However, the instructor does not want to create barriers to exercising.
